Abstract

Die Qualität der Versorgung, ihre Sicherung, Verbesserung und Incentivierung sind das erklärte Ziel der Krankenhausgesetzgebung der letzten Jahre. Für die Diskussion über Bewertung, Vergütung und Kommunikation von Qualität im medizinischen Versorgungsgeschehen liefert der Qualitätsmonitor 2019 weitere Impulse. Das Werk greift jährlich relevante Themen aus stationärer und sektorenübergreifender Versorgung auf. Die Fachbeiträge gehen dabei der Frage nach, wie sich das Versorgungssystem unter Qualitätsaspekten gestalten lässt und wie Versorgungsqualität transparenter gemacht werden kann. Weiterhin werden Strukturanforderungen einer qualitätsorientierten medizinischen Versorgung diskutiert und die wissenschaftliche Evidenz zur Gestaltung relevanter Versorgungsbereiche zusammengefasst. Eine weitere Sektion zeigt Eckdaten zur Versorgungsstruktur und Qualitätsdaten der deutschen Krankenhäuser. Der Qualitätsmonitor 2019 unterstützt fundiert die Qualitätssicherungsdebatte in Deutschland.

Abstract

Cancer care delivery refers to the multiple layers of the health care system that interact to affect outcomes for patients with cancer and the quality of that care. The factors included in the care delivery system that potentially alter outcomes include social dynamics, financing systems, organizational structures and processes, health technologies, provider and individual behaviors. Because women’s health care has its own unique challenges, the intersection between cancer care delivery and women’s health is to be examined in this Frontiers in Oncology issue. The unique opportunities and challenges of improving the health care system for women with breast and gynecologic cancers are to be explored in depth. We will visit many topics of cancer care delivery with the unique perspective geared towards the care of women’s malignancies.

Abstract

Trata do papel dos movimentos sociais em saúde no processo de reformulação da política de saúde, plasmado na Constituição de 1988 e denominado de Reforma Sanitária brasileira. Os percalços da sua implementação são analisados no contexto do processo de democratização do País ocorrido nos últimos 10 anos. A autora contribui para a interpretação da problemática da saúde no Brasil contemporâneo, sem a qual não se pode falar em democracia e cidadania. Através da leitura deste trabalho, os cientistas políticos e os profissionais de saúde poderão refletir acerca do lugar que ocupam no sistema e nos serviços de saúde, bem como das articulações de suas representações com a política de saúde.

Abstract

This report reviews the quality of health care in Sweden. It begins by providing an overview of the range of policies and practices aimed at supporting quality of care in Sweden (Chapter 1). It then focuses on three key areas particularly relevant to elderly populations: strengthening primary care in Sweden (Chapter 2), better assurance for quality in long-term care (Chapter 3), and improving care after hip fracture and stroke (Chapter 4). In examining these areas, this report highlights best practices and provides recommendations to improve the quality of care in Sweden.

Abstract

This report reviews the quality of health care in Italy, seeks to highlight best practices, and provides a series of targeted assessments and recommendations for further improvements to quality of care. Italy’s indicators of health system outcomes, quality and efficiency are uniformly impressive. Life expectancy is the fifth highest in the OECD. Avoidable admission rates are amongst the very best in the OECD, and case-fatality after stroke or heart attack are also well below OECD averages. These figures, however, mask profound regional differences. Five times as many children in Sicily are admitted to hospital with an asthma attack than in Tuscany, for example. Despite this, quality improvement and service redesign have taken a back-seat as the fiscal crisis has hit. Fiscal consolidation has become an over-riding priority, even as health needs rapidly evolve. Italy must urgently prioritise quality of its health care services alongside fiscal sustainability. Regional differences must be lessened, in part by giving central authorities a greater role in supporting regional monitoring of local performance. Proactive, coordinated care for people with complex needs must be delivered by a strengthened primary care sector. Fundamental to each of these steps will be ensuring that the knowledge and skills of the health care workforce are best matched to needs.

Abstract

This review of health care quality in Denmark examines policies related to quality and includes chapters covering primary and integrated care, hospital specialisation and equity. It finds that with a dense array of disease- and service-focused quality initiatives, and with information on the quality of care stored in separate data repositories, Denmark needs to create effective links and synergies between them to drive up quality in the healthcare system as a whole, rather than in disconnected elements. Primary care will be central in meeting Denmark’s future healthcare challenges of an ageing population with multiple chronic conditions. Therefore, an urgent need is to create a national vision of how a modernised primary care sector will fulfill this new coordination role. National standards, clinical guidelines, accreditation of clinical pathways and targeted financial incentive programmes could support this role, along with more transparent and formalised continual professional development. To facilitate quality improvement from the ambitious hospital rationalisation, Denmark should collect and disseminate data on the quality of individual physicians as well as the hospitals. Undergraduate training and medical research should be reviewed in light of the new service arrangements.  Close surveillance will be needed to monitor whether certain patient groups forego healthcare because travel times to providers are too long. Limited data availability complicate Denmark’s ability to monitor its commitment to equitable healthcare. There is an urgent need for renewed action to tackle risk factors of chronic ill-health that disproportionately affect low-income groups. Better information on the impact of user-charges on unmet need in low-income groups is needed.
